At the Near Care Team, you will have an independent and varied job where you collaborate with several professions to create safe, person-centered, and cohesive care at home.

The Near Care Team in Eskilstuna works with person-centered care and support for elderly people with multiple chronic conditions living in ordinary housing. The operation is part of the development towards future near care and is based on close cooperation between nursing assistants, nurses, rehabilitation staff, and social workers.

RESPONSIBILITIES

As a nursing assistant in the Near Care Team, you work in the client's home and are an important part of the multiprofessional team. The role involves significant personal responsibility while maintaining close cooperation with colleagues from different professions.

You are responsible for planning, implementing, and following up on interventions based on the client's needs and conditions. The tasks include, among other things:

• Nursing care according to the Social Services Act (SoL).
• Delegated health and medical care interventions.
• Assignment as a permanent care contact.
• Documentation according to current guidelines.
• Collaboration with clients, relatives, and other professions.

The work is based on a person-centered approach where the client's needs, resources, and participation are in focus. You contribute to creating security and continuity while showing respect for the client's and relatives' privacy.

Working hours are scheduled during days, evenings, and weekends. Driving a company car is included in the position.

QUALIFICATIONS

We are looking for someone who:

• Is educated as a nursing assistant.
• Has proof of a protected professional title from the National Board of Health and Welfare or is covered by transitional rules.
• Has at least two years of current experience as a nursing assistant.
• Has experience working with the elderly.
• Has good knowledge of Swedish, both spoken and written.
• Is accustomed to documenting and using digital tools via computer and phone.
• Has a B driving license.

It is meritorious if you have experience with:

• Work with elderly people with multiple chronic conditions.
• Palliative care.
• People with cognitive impairment.
• Home care or similar operations.

Information about protected professional title:

To be considered for the position of nursing assistant, you need to present proof of a protected professional title from the National Board of Health and Welfare.

If you are already employed on a permanent basis in Eskilstuna Municipality, you need to instead submit an employment certificate showing title, type of employment, and duration of employment, which proves that you are covered by the transitional rules for permanent employees as of July 1, 2023.

Salary: Monthly salary.
Employment type: Permanent employment.
Duration: Permanent, start date: according to agreement.
