The Role of Nurse in a Combined Position

Bra Liv Sävsjö Health Center, together with Sävsjö Municipality, now offers a unique combined position where you work across boundaries between the health center and the municipality. This new position allows you to develop collaboration across borders with a focus on continuity, a holistic view, and safe care for patients and their relatives.

You will be employed at Bra Liv Sävsjö Health Center, and your work will be divided between the health center and municipal home health care.

You will work with the investigation and follow-up of cognitive impairment, collaborate in interdisciplinary teams, and be an important link between the health center, the municipality, and other stakeholders. The work also includes educational initiatives, support to operations, as well as development and quality work.

At Bra Liv Sävsjö Health Center, an experienced and knowledgeable team works to provide our patients with good continuity and a positive experience, leading to excellent results. The health center has approximately 11,500 registered patients, 55 employees, and is part of the Bra Liv Health Centers. Your Profile

We prefer that you who apply are:

• Licensed nurse, preferably with specialist training in cognitive disorders
• Experienced in working with the elderly or with cognitive disorders.
• Experienced in driving improvements and development issues.
