We are now looking for two new colleagues in the area of Health and Medical Care. One position is for Kommunrehab, as one of our employees will retire, and another for the planning team.

Kommunrehab consists of a unit manager, administrative assistant, occupational therapists, physiotherapists, and a vision and hearing instructor. Our target group is primarily individuals over 65 years old with disabilities, but we also have a team that works within LSS and social psychiatry. We work according to a salutogenic approach to strengthen the individual's resources and create conditions for as independent a life as possible.

In the Safe Transition/Planning Team, we work to create a smooth and secure transition for clients/patients from the region to the municipality. We are a close-knit team consisting of physiotherapists, nurses, and case managers. We are now looking for an occupational therapist who enjoys juggling many tasks and working to create the best possible conditions for our clients/patients.

JOB RESPONSIBILITIES

We are looking for someone who is passionate about helping others achieve a more independent and active daily life. In the role of occupational therapist, you will have a varied and meaningful workday where you work closely with people and make a difference in their lives.

Your main responsibilities at Kommunrehab include:

• Assessment of activity ability in patients in regular and special housing.
• Prescription of assistive devices and writing certificates.
• Guidance and training of care staff.
• Creating individually tailored solutions to support patients' independence and quality of life.
• Collaborating with colleagues and other professionals to provide the best possible support to the municipality's residents.

Your main responsibilities in the Planning Team include:

• Working closely with a team of physiotherapists, nurses, and case managers to plan and implement discharges for clients/patients from the region to the municipality.
• Being responsible for planning health and medical care interventions, ensuring that clients/patients have a safe and secure discharge.
• Planning through the web-based systems Cosmic Link.
• Conducting planning with clients/patients both physically at Enköping Hospital and digitally from Akademiska Hospital.
• Being the spider in the web and keeping track of many different contacts and cases simultaneously.
• Coordinating and following up on cases to ensure that all processes run smoothly.

QUALIFICATIONS

We are looking for someone who has a genuine commitment to occupational therapy and enjoys working both independently and in a team, and is comfortable building and maintaining many contacts.

To succeed in this role, we see that you have:

• Licensed occupational therapist with an approved degree.
• Driver's license (B) is a requirement.
• To work in our operational systems, you must have Bank ID, Freja eID, and Sith card.
• Good knowledge of Swedish, both spoken and written.
• Ability to work independently and take initiative.
• Flexibility and ability to adapt to changing circumstances.
• A holistic view where you can prioritize and make well-founded decisions.
