As a nurse in home healthcare, you will work in one or more home care areas and be responsible for a number of patients in their own homes. JOB RESPONSIBILITIES

We conduct our operations based on applicable laws, guidelines, and routines that ensure that patients receive the health and medical care they need. We work with digital signing of medications and have the ability to document our efforts mobile. We are developing our work with digital solutions and actively working to review and change our working methods towards the transition to Close Care in Sörmland.

Main job responsibilities:

• independently assess, plan, organize, perform, and follow up medical treatment and health care assignments for our patients.
• Identify and actively prevent health risks and register in quality registers.
• coordinate contacts and collaborate in teams both internally and externally to meet individual needs.
• instruct and delegate health care tasks to nursing staff and supervise students.

The work primarily takes place during the day, but weekend and evening shifts are also included.

QUALIFICATIONS

We are looking for someone who has a nursing degree and Swedish license as a nurse. If you have specialist or advanced training as a district nurse, it is an advantage, but not a requirement.

We want you to have worked for a few years as a nurse and have a pronounced interest in working in home healthcare. Experience in home healthcare is an advantage. You should have good computer skills, experience in documenting in journal systems, and be able to express yourself well in spoken and written Swedish. The job requires you to have a B driver's license and experience driving a car.

We place great emphasis on personal qualities. You are confident in your profession, take responsibility, and have a reflective approach. You are enthusiastic and have good problem-solving skills. Since a large part of the work involves supervising nursing staff, it is important that you enjoy conveying instructions to others in a pedagogical manner. As you will meet many different types of people, it is important that you are easy to collaborate with and always put the patient first.
