Care Coordinator at the Orthopedic Clinic

The Orthopedic Department 58 provides specialized and complex orthopedic inpatient care for patients aged 15 and up and is part of the VO Orthopedics at Danderyd Hospital.

Our care coordinator will go on parental leave, and we are now looking for a replacement for a one-year temporary position with a possible extension.

Job Description:

As a care coordinator, you have a key function in the department and work to optimize patient flow, reporting directly to the unit manager. Through close collaboration and communication with the nursing team in the department, you will have a comprehensive view of everything from the influx from the emergency department, admissions, current care needs, movable patients, and discharges. Daytime Monday-Friday, from 07:00 to 15:30. Some clinical work may occur.

Main Responsibilities:

• Collaborate closely with the care team and doctors to have a comprehensive view of patients' current care needs, infections, admissions, discharges, and movable patients.
• Be the contact between the emergency department and the hospital's other wards and department coordinators.
• Answer the ward phone and help patients navigate the care chain.
• Responsible for optimizing the flow to and from the clinic.
• Report to the unit manager/care manager and have a coordinating function in case of capacity shortages.
• Responsible for initiation and follow-ups.

Qualifications:

Assistant nurse or nurse with previous experience in emergency care. Previous experience as a coordinator or similar role is an advantage.

You are a good representative of the Orthopedic department at the hospital, communicative and diplomatic while having the ability to prioritize.

Our Employment Offer

About the Business

The Orthopedic area at Danderyd Hospital is a 24/7 operation with over 300 employees. We provide both acute and elective care and meet patients at our orthopedic outpatient clinic, at the orthopedic section in the hospital's emergency department, and at our two orthopedic inpatient wards. We operate in central surgery with our own surgical staff and also conduct day orthopedic surgical operations. Our operations also include a comprehensive paramedical section.
