We are now looking for a Care Navigator - a central function that helps patients navigate the healthcare system and ensures they receive the right care at the right time.

The Unit for Production and Capacity Management is a strategic function within Region Västernorrland. We work to ensure that healthcare resources are used optimally and that the flow of healthcare production operates efficiently. In close collaboration with operations and decision-makers, we develop systems for governance, monitoring, and improvement, contributing to a sustainable healthcare structure for the entire region.

The position is a permanent full-time employment with a start date according to agreement. The location of the position can be discussed, but the role has a

regional mandate and you need to be able to work flexibly across the county's various operations.

Care Navigator for the Unit for Production and Capacity Management Job Responsibilities

As a Care Navigator, you have an important role in supporting patients through the various stages of healthcare. You help them understand their rights, guide them to the appropriate level of care, and ensure they receive the care they need. The role involves both operational and strategic work, where you collaborate with healthcare providers, authorities, and other stakeholders to create a smooth and efficient healthcare process.

Your main job responsibilities include:

• Supporting patients in navigating the healthcare system and understanding their rights.
• Collaborating with healthcare providers and authorities to ensure a smooth healthcare process.
• Monitoring and analyzing healthcare flows to identify areas for improvement.
• Contributing to the development of routines and working methods for more accessible healthcare.

We would like you to have a good understanding of the healthcare system and patients' needs, as well as an interest in development work and digital tools.

We are looking for someone who

• Has experience working in healthcare, social work, or similar fields
• Has good communication skills and can express themselves well in both speech and writing
• Has good knowledge of the healthcare system and patients' needs
