The Mobile Team Primary Care operates in nine locations in Skåne and is perceived very positively. Since June 2024, the team has been in Hässleholm and covers the municipalities of Hässleholm, Osby, and Perstorp.

The healthcare providers involved include Hässleholm Hospital, Läkarbil Skåne, ambulances, private and public health centers, and other healthcare providers in the area, such as the care and nursing administration in each municipality.

The Mobile Team Primary Care offers safe, clear, and accessible care for patients who need healthcare at home instead of hospital care, where the team can assess, plan, prescribe, and perform interventions. The care is specialized based on each individual patient's needs, and the goal is to provide patients with qualified care both medically and psychosocially.

The team consists of physicians, nurses, social workers, and medical secretaries.

RESPONSIBILITIES

The position involves independent work where you, both individually and in a team, make home visits to patients in the various municipalities. You lead and actively participate in planning around the patient's care together with other professionals and collaborate with other healthcare providers. The patient receives medical assessment and treatment that would otherwise have been performed in a hospital. In cases where patients may need hospital care, we strive for direct admissions to the ward. Participation in the development and improvement work of the organization is, of course, also part of your responsibilities.

QUALIFICATIONS

We are looking for you who are a licensed physician with the authority to practice within Swedish healthcare and have specialist competence in general medicine, internal medicine, or other specialist competence deemed relevant. Language skills in Swedish corresponding to at least level C1 according to the Council of Europe's scale are required. A valid B driver's license is also required, as care is primarily provided in patients' homes. Experience from similar work is an advantage.

You have good professional skills, are secure in your professional role, and have the patient in focus with the ability to see the individual needs of the patient. Part of the work involves support and conversations, which is why you must be communicative and able to meet people in different stages of life. You have a flexible approach to solving the day's work layout. Quality and improvement work and close collaboration with the team are a given. Great emphasis is placed on personal suitability.
